Retatrutide vs tirzepatide: three receptors versus two, and no head-to-head result yet.

Retatrutide and tirzepatide are both Eli Lilly medicines that act on the GIP and GLP-1 receptors, and they are often compared online as if one were simply a stronger version of the other. They are not at the same stage. Tirzepatide is registered in Australia as Mounjaro; retatrutide is an investigational medicine that is not approved anywhere. This page compares how they work, their status, their trial results and their side-effect signals, and explains why every comparison so far is between different trials.

QUICK ANSWER

Tirzepatide (Mounjaro) activates two hormone receptors, GIP and GLP-1, and is registered in Australia. Retatrutide adds a third, the glucagon receptor, and is investigational and unapproved everywhere. Retatrutide's average trial weight losses have been larger, but they come from different trials, populations and durations. The direct comparison, TRIUMPH-5, had not reported as of 20 October 2026.

STATUS IN AUSTRALIA · AS OF 20 OCTOBER 2026
  • Tirzepatide is registered as Mounjaro. Supplied by Eli Lilly Australia as the Mounjaro KwikPen. The current CMI lists type 2 diabetes, weight management in adults with overweight or obesity, and moderate-to-severe obstructive sleep apnoea in adults with obesity. Source: Mounjaro KwikPen CMI, September 2026.
  • Mounjaro is not PBS-subsidised. Lilly is not proceeding with a PBS listing, and the listing process "has ceased". Source: PBS medicine status, current at 1 October 2026.
  • Retatrutide is not approved anywhere. The TGA says it "has not been approved by the TGA or any comparable international regulator". Lilly plans a US application in the first quarter of 2027. Sources: TGA, 14 September 2026; Lilly, 29 September 2026.
  • No head-to-head result. TRIUMPH-5 (NCT06662383) compares the two directly; no results had been reported. Source: ClinicalTrials.gov registry.
01 · Mechanism

How do retatrutide and tirzepatide work differently?

Tirzepatide is a dual agonist: it activates the GIP and GLP-1 receptors. Retatrutide is a triple agonist: it activates GIP, GLP-1 and glucagon receptors.

Medicines that act on these receptors are registered in Australia for type 2 diabetes and weight management; the how Mounjaro works explainer covers tirzepatide's dual action in more detail. Retatrutide keeps both of those actions and adds the glucagon receptor.

The balance is different, too. The 2023 Phase 2 paper reports that, compared with the body's own hormones, retatrutide is less potent at the human glucagon receptor (about 0.3 times) and GLP-1 receptor (about 0.4 times), and more potent at the GIP receptor (about 8.9 times). It is a single peptide given once weekly in trials, with a half-life of about six days. Tirzepatide is also a once-weekly injection; in Australia it comes as the KwikPen, which holds four weekly doses.

A third receptor does not automatically mean a better or safer medicine. Whether the added glucagon action changes the balance of benefit and harm is exactly what the trials, and later regulators, need to establish.

Retatrutide vs tirzepatide: how do they compare?

The biggest practical difference is status: tirzepatide can be prescribed in Australia, and retatrutide cannot. The trial rows are from different studies and are not a head-to-head comparison.

03 · Weight loss

Does retatrutide cause more weight loss than tirzepatide?

Not proven. Retatrutide's trial averages are higher, but no completed trial has compared the two directly.

TrialLengthTreatment-regimen estimandEfficacy estimand
SURMOUNT-1 (tirzepatide 5 / 10 / 15 mg)72 weeks−15.0% / −19.5% / −20.9% vs −3.1%−16.0% / −21.4% / −22.5%
TRIUMPH-1 (retatrutide 4 / 9 / 12 mg)80 weeks−17.6% / −23.7% / −25.0% vs −3.9%−19.0% / −25.9% / −28.3% vs −2.2%

The "treatment-regimen estimand" counts everyone, whether or not they stopped treatment. The "efficacy estimand" estimates the result if everyone had stayed on treatment, and gives the larger number. Comparing like with like matters: quoting retatrutide's efficacy figure against tirzepatide's treatment-regimen figure exaggerates the gap.

Even like-for-like, this is a cross-trial comparison, and it has three weaknesses. The trials ran for different lengths (72 versus 80 weeks). The populations differed: TRIUMPH-1 enrolled adults with obesity or overweight plus at least one related condition, with an average starting BMI of 40.0. And the trials were run at different times with different designs. Any of these can shift an average.

The SURMOUNT-1 result is from a peer-reviewed NEJM paper; the TRIUMPH-1 result is a Lilly topline release. A widely shared 30.3% retatrutide figure is from a 104-week extension subgroup of people with a BMI of 35 or more who tolerated their dose, not from the main trial.

04 · Side effects

How do the side effects compare?

Both mainly cause gastrointestinal effects. Retatrutide trials have also reported abnormal skin sensations and, in Phase 2, a rise in heart rate.

In TRIUMPH-1, on 12 mg retatrutide, nausea affected 42.4% of people (placebo 14.8%), diarrhoea 32.0% (13.5%), constipation 26.1% (10.9%) and vomiting 25.3% (4.8%). Dysaesthesia, meaning abnormal skin sensations such as tingling, burning or sensitivity to touch, affected 12.5% (0.9%), and 11.3% stopped because of adverse events (4.9%). The retatrutide side effects guide has the figures for every trial.

For tirzepatide, the Australian CMI is the reference. It warns that Mounjaro "may cause inflammation of the pancreas", that use with a sulfonylurea or insulin "may cause your blood sugar to be too low", and advises drinking plenty of fluids to avoid dehydration from vomiting or diarrhoea. Frequencies are covered in the Mounjaro side effects guide.

Putting the two sets of percentages side by side would repeat the cross-trial problem. Side-effect rates depend on the population, the dose schedule and how events were recorded. A head-to-head trial such as TRIUMPH-5 is the way to compare them under the same conditions.

05 · In practice

Retatrutide vs Mounjaro: what is the practical difference in Australia?

Mounjaro is a registered medicine with Australian product information. Retatrutide outside a trial is an unapproved product of unknown content.

Mounjaro
Prescribed by a doctor, dispensed by a pharmacy, with a TGA-approved CMI that sets out doses, missed-dose rules and warnings. It is on private prescription only, as it is not PBS-subsidised. See Mounjaro doses.
Retatrutide
Lilly says it is "legally available only to participants in Lilly's clinical trials". The TGA says it is unlawful to import, advertise, manufacture and supply unapproved peptide products in Australia.
Counterfeits
The TGA tested a vial sold as retatrutide after a person was hospitalised with a torn oesophagus. It contained no retatrutide, only undeclared semaglutide at about 8 times the registered strength.
Liver injury
Victoria's Chief Health Officer has linked unapproved products labelled Retatrutide, also sold as "Reta", "R-10" and "R-20", to six cases of acute liver injury (alert of 19 June 2026).
06 · Head to head

What will the TRIUMPH-5 trial tell us?

TRIUMPH-5 is designed to compare retatrutide and tirzepatide directly, and it has not reported.

The trial is registered as NCT06662383 on ClinicalTrials.gov and as 2024-511450-49-00 in the EU Clinical Trials Information System. It compares retatrutide with tirzepatide in adults with obesity, and its primary outcome is the percentage change in body weight at week 80. The two registries give different enrolment figures and completion dates, so we do not quote either.

Until it reports, statements that retatrutide "beats" Mounjaro rest on cross-trial comparison. When results appear, the questions worth checking are which estimand is being quoted, the doses compared, the side-effect and discontinuation rates in each arm, and whether the results have been peer reviewed.

For anyone weighing up medicine for weight today, the relevant comparison is between options that are registered in Australia. The weight-loss medication in Australia guide and Mounjaro vs Wegovy explain those, and a GP can advise on whether any medicine is appropriate.

Common questions
What is the difference between retatrutide and tirzepatide?

Tirzepatide activates two hormone receptors, GIP and GLP-1. Retatrutide activates those two plus a third, the glucagon receptor. Tirzepatide is registered in Australia as Mounjaro; retatrutide is investigational and not approved anywhere. Both are made by Eli Lilly.

Is retatrutide stronger than Mounjaro?

That has not been shown. Retatrutide's trial averages are higher, for example 28.3% weight loss on 12 mg over 80 weeks in TRIUMPH-1 against 22.5% on 15 mg tirzepatide over 72 weeks in SURMOUNT-1, but these are different trials. The head-to-head trial, TRIUMPH-5, has not reported.

Is there a head-to-head trial of retatrutide and tirzepatide?

Yes. TRIUMPH-5 (NCT06662383) compares retatrutide with tirzepatide in adults with obesity, with percentage weight change at week 80 as the primary outcome. No results had been reported as of 20 October 2026.

Can I switch from Mounjaro to retatrutide?

Not lawfully outside a clinical trial. Retatrutide is not approved in Australia, and the TGA warns that any retatrutide product accessed outside clinical trials could be counterfeit. Questions about your current medicine belong with your prescriber.

Do retatrutide and tirzepatide have the same side effects?

Both mainly cause gastrointestinal effects such as nausea, diarrhoea, constipation and vomiting. Retatrutide trials have also reported abnormal skin sensations (dysaesthesia) and, in Phase 2, a dose-dependent rise in heart rate. A fair comparison of rates needs the head-to-head trial.

Is Zepbound the same as Mounjaro?

Both contain tirzepatide. Zepbound is the US brand name for weight management, approved by the FDA on 8 November 2023. In Australia, tirzepatide is supplied under the brand name Mounjaro.
